Transcend partners with people across Northern Virginia to provide sponsored residential services to children or adults with developmental and intellectual disabilities. Our sponsored Residential Providers or Host Home Providers promote community inclusion by opening their homes to one or two individuals with intellectual or developmental disability, providing a safe and comfortable home environment. Transcend acts as a liaison between the individual and the host home provider.
We are centered on creating safe, stable, and nurturing homes for children and adults who depend on our expertise. We specialize in careful and thoughtful residential coordination, ensuring a harmonious match between children or adults with disabilities and dedicated caregivers. Our residential service coordinators will carefully match each client with compassionate providers who will provide a family-style, community-based living arrangement for children and adults with IDD, where trained family providers open their homes to offer support for daily living, personal care, and community integration. We offer personalized services, focusing on an individual's specific needs, goals, and preferences to promote independence in a safe and stable home environment.

Transcend Residential Services works to find a home that is the right fit for each client, considering their personality, needs, and location preferences. Individuals live in a private residence with a trained provider, creating a more personalized, and one-on-one care model.
Programs use an Individual Support Plan (ISP) to tailor services to each person's needs, which can include assistance with daily living skills, personal care, and medication management.
We focus on understanding challenging behaviors and teaching new skills to improve quality of life. We train all sponsored home providers to increase socially appropriate behaviors by modifying the environment and teaching replacement skills using positive reinforcement
Transcend sponsor providers help individuals participate in community activities, pursue educational or occupational goals, and build relationships. Participation in preferred leisure activities is also prioritized
All sponsors are trained by Transcend to offer services in a way that is compliant with state regulations, with ongoing training provided.
Transcend utilizes a team-based approach with professionals providing support and supervision to sponsor parents to ensure quality care.
